چکیده

A 19-year-old woman had three episodes of left facial paralysis, each with complete resolution. The initial computed tomographic (CT) scan was interpreted as normal (Fig 1A). The patient returned 1 year later with recurrent facial paralysis. Magnetic resonance (MR) imaging showed a mass at the geniculate turn (Fig 1B). Retrospective review of the initial CT findings revealed expansion of the distal labyrinthine segment of the facial nerve and geniculate turn. The bony margins were indistinct with suggestion of faint intratumoral bone. The presumptive diagnosis was an “ossifying” hemangioma. At surgery, a fibrotic tumor was present at the geniculate turn, which was excised along with the tumor. A greater auricular nerve graft was placed. At pathology, the mass was noted to compress the facial nerve. Microscopically, it demonstrated psammoma bodies with plump elongate spindle cells with a syncytial growth pattern. Immunohistochemical features were also consistent with the diagnosis of meningioma. Strong positive results were obtained with epithelial membrane antigen and vimentin. Stains for low-molecular-weight cytokeratin and factor VIII revealed normal levels. The patient had not regained any appreciable facial nerve function by 2 years after surgery.

A new entity in the differential diagnosis of geniculate ganglion tumours: fibrous connective tissue lesion of the facial nerve.

Differential diagnosis of geniculate ganglion tumours includes chiefly schwannomas, haemangiomas and meningiomas. We report the case of a patient whose clinical and imaging findings mimicked the presentation of a facial nerve schwannoma.Pathological studies revealed a lesion with nerve bundles unstructured by intense collagenisation. Consequently, it was called fibrous connective tissue lesion ...

Idiopathic, herpetic, and HIV-associated facial nerve palsies: abnormal MR enhancement patterns.

PURPOSE To determine specific criteria that can be used to define normal versus abnormal MR contrast enhancement of the facial nerve. METHODS Twenty-three patients with acute unilateral inflammatory peripheral facial nerve palsy were examined on a 1.5-T MR using multiplanar T1-weighted spin-echo sequences before and after injection of gadopentetate dimeglumine. These MR patterns were compared...

High-resolution MR imaging of juxtasellar meningiomas with CT and angiographic correlation.

The purpose of this study was to compare the relative merits of MR imaging, CT, and angiography in the preoperative evaluation and postoperative follow-up of patients with juxtasellar meningiomas. High-resolution MR studies in nine patients with juxtasellar meningiomas were evaluated and compared with CT and angiography. The techniques were compared for the evaluation of suprasellar, parasellar...

Anterior Displacement of the Geniculate Ganglion.

We present the case of a 34-year-old Japanese woman with cholesteatoma of the middle ear. During the operation, this patient showed an unusual position of the geniculate ganglion. We reviewed the computed tomography (CT) images targeting the ear of the present case after the operation. We found that the shortest ranges from the ampullated end of the superior semicircular canal to the geniculate...

Angiomatous Meningioma: CT and MR Imaging Features

giomas on computed tomography (CT) and magnetic resonance imaging (MRI) are easily identified and hence, easy to diagnose (1). However, certain meningioma subtypes have unusual imaging findings, making it difficult to diagnose them as well as determining the appropriate treatment protocol. This is particularly true for rare meningioma subtypes, which include the angiomatous meningioma, which sh...
